Mandatory Reporting 

Every child and youth death or serious injury needs to be reported to the Office of the Child and Youth Advocate. Please call 902-368-5630 or email voiceforchildren@ocyapei.ca to report.

Reviews and Investigations

The Child and Youth Advocate may review or investigate a child or youth’s death or serious injury. The reasons for this are:


•    to identify systemic issues; 
•    to identify potential enhancements to practice or resources that could benefit children and youth who receive reviewable services in the future;
•    to make recommendations to improve systems that serve children and youth; and 
•    to make recommendations about how to prevent child and youth serious injuries and deaths in the future

Reviews and investigations do not determine legal responsibility and are conducted in line with the principles of respect, integrity, collaboration, and balance (more details here).
